- What is MD Monitor about?
MdMonitor is one part of an advanced and complete patient health monitoring and medical record system. This can help you to keep in control of your health by allowing you to team up with your health care providers.
What does it allow you to do?
You can monitor your blood sugar levels, weight and your blood pressure etc. with easy-to-view graphs.
You have access to all your medical information.
You can set appointments, reminders to be at the appointment and also set reminders to take your medicines etc.
MdMonitor will alert you by email or text message to your cell phone.
You will be able to enter your readings by cell phone, or regular phone as well as online.
- How do I sign up?
If you need glucose meters or other equipment and/or wish to register with the Disease management system, please call PDS Health at (800) 440 2417 and ask about our programs.
- If you are a patient
Please register as a patient first. Then log in as a patient. Any caretaker who is in the system, can also let you register from their application. In that case, you automatically accept him or her as one of your caretakers and your information will be available for them to view and change. The first time you log in you need to fill out your medical history which is strictly confidential .
Your information will not be transferred anywhere and is encrypted.
After you have filled out the information you can start using the MDMonitor by simply entering your data in the program.
If you want your physician , case manager , certified diabetes educator or any other caretaker to be able to see the information you need to allow them to do so by accepting each as authorized caretaker on the form.
If your doctor, or other caretaker is not registered please ask them to register.
- If you are a Care taker, and/or represent a Hospital, clinic, physician CDE , Case manager or other health professional.
Please register as a caretaker. After that you can log in as a caretaker. If you represent a clinic, hospital, doctor’s office or any other entity with several health professionals, you may permit them to sign in under your umbrella login name. Each staff member gets his/her own login code if they wish. THEY HAVE TO LOGIN FROM CARE TAKER – PHYSICIANS.
A caretaker can sign in new patients and automatically accept them as one of your patients from the New patient icon and the patient’s information will be available to view, analyze and change.
The first time you log in you need to accept a person as your patient on the form. After that you can see the information and start analyzing the patient condition and plan their care. Patient information will not be transferred anywhere and is encrypted.
Can I sign up my own patients?
When you click on the icon New patients , you will get a screen that allows a patient to automatically sign is as your patient. You may register patients in your office.
How do my patients register with me
Each patient has to accept a caretaker and the care taker has to accept the person as a patient .
If you are a vendor
Anyone who wants to sell their services or products to patients, hospitals, caretakers, manufacturers, wholesalers, or retailers can sign up and upload information and prices about their services.
- Can I compare services and product prices?
Yes , a patient sees vendors services and prices and MdMonitor.com allows you to compare them automatically.
- Can a physician view X-rays?
Yes- use the Teleradiology feature. Scan in an X-ray and upload it to MdMonitor.com, then a physician can email the access code for a particular X-ray to a radiologist who then can interpret it.
Go to List Patients. On the right hand side you can see Radiology – click on View/Edit and you will see what X-rays are uploaded. Click on the picture and you will be able to analyze the Xray.
Scanning X-rays
Use the following settings:
Gray scale
Increased or full brightness
Medium contrast
And highest resolution approximately 600pi
- What is NAG?
NAG stands for Needs Assessment Generator. It’s a Telephony feature that calls the selected patients with optional questions and assesses their needs for blood glucose or blood pressure monitoring etc.
Click on NAG and Edit/View the questions you want to be asked of the patient.
- Why is the information in different colors?
The program differentiates who has entered the information by colors. Black is information entered by caretaker and blue is data entered by a patient.
- Can a case manager run reports?
Yes – The following reports are available:
Weight Level : Lbs Displays the patients with Weight level greater than given weight
Blood Pressure : Displays BP Level and BP History
Cholesterol : Displays Cholesterol Level and Cholesterol History
Triglycerides: Displays Triglycerides Level and Triglycerides History
Blood Glucose Level : Displays the patients with Blood sugar level within given range
Hemoglobin Level : Displays the patients with A1c level within given range
Smoker : Displays Tobacco Type, Tobacco usage per day and onset of Tobacco use
Treatment Procedure Code : Displays Icd-9 Code, Surgery, Performed By identified caretaker, Credentials, Clinic Name and Date
- What is patient discharge?
When a patient is discharged from a Hospital the Discharge manager can use this feature for the PRI and to send orders to vendors online.
